Admission Process

What is needed to start ABA services with us:

1.  Initial Assessment (Admission):

 

• Copy of Health Insurance card for the client (We currently accept Full Medicaid and Cigna insurance).

• Intake to consent to treatment (provided by us)

•  Physician’s written order for ABA services dated less than a year ago.

•  Medical necessity letter for ABA services dated less than a year ago.

•  Comprehensive Diagnostic Evaluation: The CDE must have been conducted by licensed practitioners qualified to assess child developmental disorders such as:

  • Developmental Pediatrician

  • Child Neurologist

  • Child Psychologist

  • Child Psychiatrists

If ABA services are to be conducted in a public-school setting, we will need current IEP for client.​

2. Transfers:

If the case is a transfer from a previous agency we will need the following:

• Latest behavioral treatment plan

• Filled and signed by caregiver Change of provider Form (We can provide you with a blank copy).

• Copy of child’s health insurance card

• Intake to consent to treatment form (We provide you with a blank copy of this form).

• Physician’s written order for ABA services dated less than a year ago.

• Medical necessity letter for ABA services dated less than a year ago.
